Many people in underserved communities experience a gap in the essential needs they have versus those required to thrive. In
partnership with the Urban Neighborhood Initiative (UNI), KC Health Collaborative is working to create a Community Care Network to help patients meet these essential needs.

The Community Care Network will connect doctors and hospitals with community-based organizations to help patients and support whole-person care.
The organizations will coordinate patient services and track when and how they receive support. By combining quality healthcare with community services, we are working to improve health outcomes in the UNI neighborhoods.
